When an in-person request is made, the only information a custodian may request is age, identification, description of the requested records, and signature in a register. The Shreve Memorial Library does not employ these requirements, save the description of requested records.

For written requests, the Shreve Memorial Library requires at least one form of contact information, as well as a specific description of the requested records.
